The DS1500 form, now replaced by the SR1 form, supports benefit claims for individuals nearing end of life. The form is used to expedite claims for terminally ill people who are applying for certain benefits under special rules. It provides current medical/clinical diagnosis, prognosis, and treatment applicable to the person in question.
The patient (adult or child) is “nearing end of life,” when they are likely to have less than 12 months to live. The “nearing end of life” definition across welfare and health services allows clinicians to discuss the patient’s and preferences, including any financial issues. If the claimant meets the eligibility criteria, then, he/she, can make a fast-track claim for some (or all) of the following benefits:
- Personal Independent Payment (PIP)
- Universal Credit (UC)
- Employment and Support Allowance (ESA)
- Disability Living Allowance (DLA) for children
- Attendance Allowance (AA)
The health professionals (Clinician, GP, Consultant, or hospice doctor) can complete the SR1 form if they believe their patient has a progressive disease and may not survive beyond 12 months. However, if the patient survives beyond twelve months, there are no negative consequences for the patient or the health professional who completed the form.
